Navigate to: "https://addonhealthcare.com/"

2. Click on Book Appointment

Click on the button labelled "Book appointment"

3. Enter the Details

a. Enter the Patient details like Name, Email, Phone Number

b. Enter the test details in the Message box

c. Click on Submit

4. Appointment Confirmation
