3. Enter the details

a. Select the test name from the drop down list

b. Enter the mobile number, Name of the Patient, Test date and Email Id in the given blocks

4. Make an Appointment

a. Click on the button labelled Make an Appointment

b. You will see a thank you not at the bottom and the Anand Imaging Center team will get back to you.
