3. Enter the Details

a. Enter the patient details like Name, Email Id, Subject ( Test category / Department if known), Mobile number

b. Choose the Doctor's prescription file from your PC/ Mobile which ever you are using to attach in Doctor's Prescription option.

4. Click on SEND

Click on the button labelled "SEND"

5. Appointment Confirmation
