2. Click on BOOK AN APPOINTMENT
Click on the button labelled "BOOK AN APPOINTMENT"
3. Fill the BOOK AN APPOINTMENT Form
1. Enter your Name
2. Enter your Email Id
3. Enter your Phone Number
4. Select the Appointment Date
5. Select the required service from the list
6. Select the Visit as "Center Visit"
7. Enter the test name in the message box
8. Click on the button labelled "SUBMIT"
