Navigate to https://www.phdiagnosticcentre.com/

2. Hover on LOCATE CENTER

1. Hover your mouse over the LOCATE CENTER button to see the list of locations

2. Click on any one of the locations and wait to load next page

3. Click on Find Test

Click on the button labelled "Find Test"

4. Enter your test in the Search box

Search for the test using the search box

5. Click on SUBMIT

1. Enter your Name

2. Enter your Email Id

3. Enter your Mobile Number

4. Select the Center from the list

5. Enter the Message (appointment date & time)

6. Click on the button labelled "SUBMIT"
