2. Fill the BOOK YOUR TEST FORM

1. Enter your Name

2. Enter your Email Id

3. Enter your Mobile Number

4. Select the Appointment Date

5. Select your State

6. Select your City

7. Enter the Test Name (Covid 19 RT PCR test)

8. Click on Choose File to attach the doctor's prescription

9. Enter the Verification Code ( as shown in the image)

10. Click on the button labelled "SUBMIT"
