Navigate to https://www.lalpathlabs.com/

2. Click on the yellow circle

1. You can view 11 yellow circles

2. Click on the 7th yellow circle

3. Click on KNOW MORE

Click on the button labelled "KNOW MORE"

4. Click on REQUEST A CALL

Click on the button labelled "REQUEST A CALL"

5. Fill the Home Collection Query Form

1. Enter the Patient Name

2. Enter your Email Id

3. Select the Patient City

4. Click on Choose File to attach the doctor's prescription ( make sure file size is below 5MB)

5. Enter your Mobile Number and click on send OTP

6. Enter the OTP

7. Click on the button labelled "Submit"

6. Appointment Confirmation
