3. Fill the BOOK AN APPOINTMENT Form

1. Enter your Name

2. Enter your Email Id

3. Enter your Phone Number

4. Select the Appointment Date

5. Select the service as "RT PCR detection of SARS - CoV2 RNA(COVID 19)"

6. Select the Visit as "Center Visit"

7. Click on the button labelled "SUBMIT"
