4. Fill the nCOV2 Requisition Form

2. Enter the Patient Age

3. Choose the number of people to be tested from the list

4. Enter your Phone number

5. Enter your Address

6. Select Which sample collection centre you would like to visit from the list

7. Enter your Email ID

8. Click on the button labelled "Submit"
