3. Click on Home Sample Collection
Click on the button labelled "Home Sample Collection"
4. Fill the Home Sample Collection Form
1. Enter your First Name and Last Name
2. Enter your Email
3. Enter your Phone Number
4. Select the Gender
5. Enter your DOB (Date Of Birth)
6. Select the City
7. Select the Location
8. Select the Appointment Date
9. Select the Time Slot
10. Select the Test from the list
11. Click on the button labelled "SUBMIT"
