2. Select the Gender
3. Enter your Date Of Birth
4. Enter your Email Id
5. Enter your Phone Number
6. Select the Type of Appointment (Home Collection)
7. Enter your Address
8. Enter your Pin code
9. Enter your City
10. Enter your State
11. Select the Appointment Date & Time
12. Click on the button labelled "Submit"
5. Appointment Confirmation
