3. Fill the BOOK AN APPOINTMENT Form

1. Enter your Name

2. Enter your Phone Number

3. Enter your Email Id

4. Mention clinic preference as "Home Visit"

5. Enter the test details in the Message box

6. Click on the button labelled "SEND"

1. Contact the number provided

2. Provide the patient details, address, mobile number, test details and preferred date & time to the operator

3. Operator will schedule an appointment and you will receive an Appointment Confirmation message
