2. Click on Book Home Visit
Click on the button labelled "Book Home Visit"
3. Fill the Home Visit Form
1. Enter your Name

3. Enter your Mobile Number
4. Enter your Address and Landmark
5. Select the Test type
6. Enter the Message (appointment date & time)
7. Click on Choose Files and upload the doctor prescription
8. Click on the button labelled "Submit"
