2. Click on Home Collection
Click on the button labelled "Home Collection"
3. Enter the Details
a. Enter the Patient details like Name, Age, Address with Land Mark
b. Enter Email Id and Mobile number
c. Enter the Symptoms if any and Referred by self/ Doctor
d. Enter the Preferable date and time of Collection
4. Select the Test Names
Select the List of Test Names by clicking on Home Collection Test List
5. Click on Submit My Request
Click on the button labelled "Submit My Request"
6. Appointment Confirmation
