3. Click on HOME COLLECTION

4. Click on Submit

1. Enter your First Name

2. Enter your Age

3. Enter your Address

4. Enter your Email Id

5. Enter your Phone Number

6. Click on Choose File to upload the doctor prescription

7. Click on the button labelled "Submit"
