2. Enter the Age
3. Select the Gender
4. Enter the Contact Number
5. Enter your House No, Society
6. Enter your Street Name
7. Enter your Pincode
8. Enter your Locality
9. Enter your City
10. Select the Payment Mode as "Pay cash at visit"
11. Click on the button labelled "CONFIRM & PROCEED"
