2. Click on BOOK HOME VISIT
Click on the button labelled "BOOK HOME VISIT"
3. Fill the HOME VISIT BOOKING Form
1. Select the Branch (area which is near to you)
2. Enter your Name
3. Enter your Email Id
4. Enter your Mobile number
5. Enter your Address
6. Select your Area
7. Select the Appointment Date
8. Select the Appointment Time
9. Enter Lab Test details ( write the test name which is required)
10. Enter the Captcha (as shown in the Black box)
11. Click on the button labelled "SUBMIT"
