Navigate to https://vision-diagnostic.com/

2. Click on DOWNLOAD REPORTS

Click on the button labelled "DOWNLOAD REPORTS"

3. Enter your Online Report Id

1. Enter your Online Report Id along with the first 4 letters of your name on the page

2. Your Online Report Id will be on your payment receipt

4. Click on Submit

2. After login select your test and download the report
