Navigate: "http://www.charakmedical.in/Default.aspx"
2. Click on Download Report
Click on the button labelled "Download Report"
3. Enter the Details
a. Select the Group Name as Patient from the drop down
b. Enter the User Name and Password as Printed on the Payment Receipt
c. Click on the button labelled "Login"
d. You can view the list of test reports
e. Select the test and download the report
