1. Open Healthcare Medical Centre and Diagnostics Website
Navigate to http://www.healthcarediag.com/
2. Click on Online Reports
a. Scroll down on the home page and hover the mouse over "Online Reports"
b. Click on the button labelled "Click here"
3. Enter Online Report ID
a. Enter online report ID with four characters of your first name as shown in the example.
b. Online report ID will be printed on the Receipt.
4. Click on Submit
Click on the button "Submit" to view/ Download the Reports.
