2. Click on Online Reports

Click on the button labelled "Online Reports"

3. Fill the SEARCH PATIENT Form

1. Enter the SID Number, Last Name, and Mobile Number on the page

2. Your SID Number as printed on the payment receipt

3. Enter your Last Name and registered mobile number

4. Click on the Show button

2. After login select your test and download the report
