4. Enter your Patient Id and Password
1. Enter the Patient Id and Password on the page
2. Your Patient Id and Password will be on your payment receipt
5. Select "Print Type" as With Attachment
Click on the "Print Type" and select With Attachment
6. Click on the Ok button

2. After login select your test and download the report
