1. Open Lotus Diagnostics Website

Navigate to http://www.lotusdiagnostic.com/

2. Click on ACCESS ONLINE REPORT

Click on the button "ACCESS ONLINE REPORT"

3. Enter your User ID and Password

1. Enter the User ID and Password on the page

2. Your User ID and Password will be on your payment receipt

4. Click on Login

1. Click on the button "Login"

2. You will now be able to see a link which will give information on status of report

3. If the report is ready, you will see a VIEW button

4. Click on VIEW button to open your reports. You will have the option to download or print
