Navigate to https://matrixdiagnostics.net/

2. Hover over the Reports

Hover your mouse over the "Online Reports" to see the options

3. Click on PATIENT ONLINE REPORTS

Click on the button "PATIENT ONLINE REPORTS"

4. Enter your WAC

1. Enter your WAC number on the page

2. Your WAC number will be on your payment receipt

5. Click on Login
