Navigate to: "https://www.healthchek.in/"

2. Click on View Report

Click on the button labelled "View Report"

3. Enter the User Name and Password

a. Select the Group name as Patient from the drop down

b. Enter the User Name and Password as printed on the Payment Receipt

c. Click on Login

d. You can view the list of test reports

e. Select the test and download the report
