3. Click on View Report
Click on the button labelled "View Report"
4. Select "Group Name" as Patient
Click on the "Group Name" and select Patient
5. Enter your User Name and Password
1. Enter the User Name and Password on the page
2. Your User Name and Password will be on your payment receipt
6. Click on Login Button
1. Click on the button "Login"
2. After login select your test and download the report
