Navigate to: "http://wellnessdiagnostic.in/"
3. Click on Online Report
Click on the button labelled "Online Report"
3. Enter User Name and Password
a. Select the Group name as Patient from the drop down
b. Enter User Name and Password as Printed on the Payment Receipt
c. Click on "Login"
